Hi, for "publication of service" where I'm putting an ad in a paper to serve someone for a restraining order, what is the format for the text of the notice that goes in the paper? In other words, what exactly do I write in the "ad."

Yes, we cannot serve the person. She has been harassing me constantly - doing stuff via fax and email, all after safely moving somewhere that we do not know. The police said her license shows a 2004 address. We have tried serving her at another place but she is well aware that we're trying to serve her, so service by publication is our only option now. So I need to know what do write in the notice.
