Regex expert and PHP expert pls help?

for example in this url,I am trying to use a regex expert and the search and replace function to convert the above url into this

$url[] = 'http://www.medmark.org/go.php?url=' . $domainshort . '&title=animalerasers.info&desc=+-+Features+free+personal+and+fully+automated+birthday+calendar'

can someone come out with a regular expression or a search and replace function?please assist thanks!

in this case it must also work as well
