Refocus is a simple printable template that allows you to lay out a handful of goals and track your adherence to those goals over the next 30 days.
Tracking goals in a concrete and visual way is an excellent motivational tool. The Seinfeldian Chain, previously covered here, is certainly a strong example of the effectiveness and popularity of the technique when used as motivation for a single goal. The ReFocus worksheet allows you to track up to five goals over a month. From the author's web site:

The end result looks like a cross between a Scantron form and Benjamin Franklin's Thirteen Virtues form. The purpose of this form is purely to give me a sense of accomplishment and to give feedback when I'm being unfocused. I still need to keep track of tasks that need to get done, but management of tasks and analysis of progress are now separated.​
Each circle is filled in completely, halfway, or not at all to indicate whether you made progress. If your goal was to eat healthy food and you ate a great breakfast and dinner but went out for lunch and ate fast food, you might, for example, fill in a half circle for that day. To the side there is space for notes.
