Really painfull Urinary Tract Infection... something is wrong.?

I have a serious case of urinary tract infection. I've had it ever since I was born pretty much. Atleast thats what I was told. But I'v had it pretty much all my life. When I was little I ever had surgery to enlarge my urethra so it wouldn't burn so much, so to cure it. It apperantly didn't work because I still get it, and I always have been, even after going to the doctor numerous times and getting medicine for it.

But now something different is happening, and it hurts so much, it's bringing me to tears. Try to bear with me when I tell you the details. It seems as though when I urinate, it happens one of 2 ways. Either its the normal amount of urine, but at the end it burns like crazy. Orrrr, it happens in short spurts. And it happens very frequently. It comes out as very little urine, but the pain that comes with it is crippling. Once I'm done, I can still feel the pain in my urethra, and it stays there, and it doesn't go away at all. And its always making me feel like I need to pee when I don't, because the burning sensation usually indicated to me that I needed to go to the bathroom. But now that the burning sensation just stays stationary in there, even after i'm done, its starting to become unbearable to deal with.
I am well aware that I have a very bad case of UTI, I always have, but these short incredibly painful spurts are somewhat new to me. Can anybody tell me whats going on???
