Php web application, where to start?

I have to build a web application and I have a php book in front of me and Dreamweaver CS3 installed in my computer. But i have no clue where to start. Should i make my computer into a server before i start? I test the Dreamweaver with a simple testing page, and the php part didn't work.

Kind of lost in there. Please help!
I have to create a web-based database. So will be using mySQL as DB.
