i made a form to collect emails and names.
it used to send me an email with the details, but not any more
heres the code:

$to = "myemailaddress";
$headers = "From: ".$email. "\r\n";
$subject = "Submitted application";

why doesnt it work anymore?
is there a way i can append the info to a .php file or something instead?
