Photo release form for minors.?

My 13 yr old son is playing soccer this spring, the coach is looking to set up a website to post actions shots and profile pix of the kids, I agreed to take the pictures and post them on the site but I told him we should get a release form and have the parents sign it, where can I get one or what should it state so I don’t get in trouble for posting.. Im not going to sell these photos just post them on the teams site,
 
If you contact me, I will send you a sample release form ... I am surprised the coach doesn't have one already approved by his attorney.
