PG in medicine frm World's Top Universities..?

Hi frnds..I'm doing MBBS-2nd yr frm India...I want do PG in medicine frm top universities like Harvard,Oxford,Cambridge,Yale,etc...how can I get admission there...any entrance exam by the respective universities..?? Is there any scholarship for international students..??
How is USMLE related to the admission..or It's just a licensing exam..??
pls clear my doubts...
