operation failed!!

I've had a problem with call waiting ever since I got my first phone with tmo 3 years ago. If I have a call on both lines and the person I'm on the line with hangs up before I switch to the other line my 8100 will say operation failed and I end up having to end the call. The same thing used to happen with my Razr. My girlfriend gets pissed because she thinks I'm hanging up on her to talk to someone else. Just wondering if this has happened to anyone else. Thanks
