Nuclear Bomb on U.S.?

Ok I have a couple of questions here. First off, how would the U.S. know that a nuclear bomb is on the way. Second of all, if a plane was on the way over with a nuke, what would the U.S. do. I mean its not like the U.S. would know theres a nuke on the plane. I mean is there really any way that if a country does decide to drop a nuke on us we can prevent it? And if the bomb is released from the plane, is there a way we could blow it up while its in the air.
