It shouldn't ask for a code. Are you sure that the phone is a Nextel and not a Boost or Southern Linc?
It does the same exact thing with mine when I try to use my wife's Boost SIM in my i90c or my i730. There is no code or anything on her SIM, I tried 000000 and 12345678, and they didn't work either.
Supposedly you don't need to unlock a Nextel to use a Boost SIM, but I haven't been able to with any of the phones I have.

I did. They just said a Boost SIM shouldn't work in a Nextel Phone, and a Nextel SIM shouldn't work in a Boost Phone. Then they pointed me to the Boost display and asked if I wanted to buy one.
