need to reapplying for nclex pn exam.so for that i need register form to applying.?

i want to give nclex pn exam again for which i need to fill reapplying exam form and pay $75/ money order.so for that i need to have retake /reapplying nclex pn examination form.can you able to send that form.i gave exam exam couple of months ago but i failed.i got result but there is no reapplying application form.i go though website which they mention but couldnt find it.i would be very thankful if you can kindly send me that request to retake nclex examination form so that i can send to bvnpt along with money order of $75/.thank you
