Need help please

My fiancee has been diagnosed with PTSD, along with anxiety and depression. He takes zoloft in the morning and some other anti-depressant at night to help him sleep. I have to get his pills set out for him morning and night or he won't take them, even with the bottles sitting out.
The big issue is the ativan. His Dr prescribed it "as needed" so if he doesn't think that it is needed he won't take it, even if I tell him that he might need a "really happy pill". A lot of times, I can hear it in his voice that he neeRAB to take one. It has gotten to where if I tell him he might want to take one, he gets upset and complains about everyone around him being idiots and so on. He has even gotten to where he has alienated himself from many of our frienRAB.
I am unable to go with him to his Dr appointments, so I usually tell him what he neeRAB to talk to the Dr about. I think he relays some concerns, but not all. 2 months ago, he was taken off arabien and celexa because of the concerns.
How can I get him to understand that he neeRAB the ativan more often than he thinks?
