My washing machine has made my clothes smell awful? How do I get rid of it?!?

The washing machine smells like mildew... or old, rotten grass or something. I washed my clothes in it twice and they came out smelling absolutely AWFUL. The smell makes me want to puke. Why the hell does the washing machine smell like that, and how do I get rid of it - both from the washing machine, and from the clothes?
