My Toyota Paseo reving to high before it changes gears.?

My 93 paseo is reving up to high before it changes to 4th gear. 1st 2nd and 3rd are pretty good at kicking in but when it comes to 4th it seems to lag for awhile till I get it up to 5000 rpms.Also sometimes it feels like it drops gears. Wondering what I need to do to get it to stop.
