my friends Mercury mystique(mistake)lol?

Every time my friend pulls up in his 1997 Mercury mistake it sounds like there is no oil in the engine. It don't knock in the lower end but it clicks, ticks, and rattles in the top end at an idle. but the car is full of good oil, runs good and it only gets driven about 20 miles a week. It has 136000 miles. So why does a car that runs good sound so bad?
