My beta fish is lying on his rocks on his side..help!!?

About a month ago he got sick looking and from the details i looked up it sounded like it was just a fungus so i started putting these tabs in with him. Since then he's been swimming around like normal but today i changed his tank the same way i've done for the past 8-9 months and now he is laying on his side on his rocks and barely moving.

Is he dying? He got a little white under his mouth so maybe it's ick? He keeps straining a bit like he's trying to swim and then his gills will move very fast and then he goes completely still for a bit before straining to swim again.
 
How big is the tank? He needs at least 2.5 gallons and it must be heated. The white around his mouth sounds like fungus. I'd add some aquarium salt to the tank, and with every water change.

When you did the water change did you add the dechlorinator? Was the water the exact same temperature?
