altenator, idler still no fix. 228,222mile Thank you to those who respond. I really appreciate the help.
I've taken the belt off and no noise. I've tried using a stethescope and no evail. I turned off a.c and noise is still there. It's constant while engine is running and gets louder with rise in engine rpm.

if your truck is equiped with A.C. your pump for the system maybe bad where the belt spins that pulley. pop the hood and listen for it while it runs. if you only hear it when drive and moving, listen carfully to where its comming from. the rear differential makes a noise like that when the gears or bearings are going bad.

As a test, while the engine is running, rub some bar soap on the belt. A friend has the same problem, so far he has replaced the water pump, tensioner, alt.and P/S pump.
They can't figure it out yet. The soap works
