My 2000 Ford Explorer is leaking transmission fluid. What could be the cause?

I noticed my Explorer was shifting hard so I checked the fluid today. It was low so added some in. The fluid would leak out as I pouring it. I think it was pouring out from the middle or back of the transmission I'm not really sure. What could be wrong with it? My vehicle is an automatic in case anyone wanted to know.
