...the possible causes? I have only witnessed a rough idle once, but my wife says that the truck has nearly stalled on her when trying to accelerate from a stop, even after the truck has had ample time to warm up. Throttle body has been pulled off and cleaned, plugs are good and have recently run fuel injector cleaner through the tank.

Any suggestions?
 
Check to make sure that the locking torque converter isn't staying locked up. The torque converter should be slipping at low vehicle speeds.
