Multiple health issues: Urinary, heart, bowels, help!!

To start, I'm a 23 year old guy, I'm 5'8" and weigh about 118 pounds.

For the past several months after a year and a half of suffering, I've been seeing a doctor and being treated for what is assumed to be prostatitis.

I've been through Bactrim and a weaker dose of Oxybutynin (Ditropan), and neither medication helped and only made me more miserable from the ungodly side effects.

I'm very concerned after seeing the urologist. First, the nurse took my blood pressure and heart rate and asked if I felt hot or dizzy. I said no, and she didn't say anything else about it.

The urologist did a rectal exam to check my prostate, which was the most painful thing I think I've ever experienced. I let out a good yell. His comments were "wow, you're pretty tender up there, it shouldn't be that sensitive".

So now he put me on another round of antibiotics, Doxycycline and prescribed Ditropan XL, which he wanted me to take at the same time, but I haven't touched the pills yet because I know just one a day (as opposed to the weaker dosage of 2 my regular doctor had me on) must be retention torture.

That stuff scares me too much to take, and if the weaker dose or even double the weaker dose which my doctor had me trying before never worked, why should I try this? It cut me down on actually being able to urinate (I had to push really hard), but it NEVER eliminated the urges, which are the primary concern for me.

Sometimes it's a muscle thing, and I can usually tell if its just my muscles going crazy or if I actually do have to go. I usually give in to my muscles anyway and go just to make sure though.

Let me just say that things like coffee or beer or even sometimes mixed alcoholic beverages NEVER used to do what they do to me now. If this was 2 or 3 years ago, I'd be fine with drinking coffee and not having to go very much after, but my god, now if I try that, forget it. Especially beer. Beer is horrifying to my bladder.

Also, there are times I get pain in my perineum, as if I've been working a muscle at the gym forever, and it really aches.

What is odd to me is that sometimes when I'm at home, I'm absolutely fine and I'll only have to go maybe 3 or 4 times a day if I've just been sitting around. Then again, it may be due to the fact I barely drink very much water or anything anymore, because if I do, I have to constantly pee, and I never drank much water or anything when all this started either.

Apart from my urinary issues, I also have random episodes during the day where my heartbeat goes crazy for no reason at all, I'll just start hearing my heart pounding really fast and sometimes drinking a glass of water helps, but sometimes it just happens, and my head and face will start feeling hot.

I'm really concerned about that.

Also, for the past couple years (up until very recently) I used to have chronic constipation, and so I would drink coffee every night just so I could defecate. I'm not sure if having coffee every night could really affect me having to pee all the time. I also used to smoke cigarettes (about a pack that lasted me 2 or 3 days at a time) so I don't know if that might have something to do with it too.

My stools are usually for the most part a bit thin and sometimes stringy, but not always.

And my last issue is that there are times when I have rectal/intestinal discomfort and pain which lasts for about an hour or two, which happens every couple days usually. Like I'll have aching in the rectal/anal muscle and feel a general discomfort in my intestines.

Also over the past year, people have noted that I've lost a good amount of weight (about 10 pounds or so), but I've been gaining some back since I eat more often. I used to be 125-130 pounds when I was completely healthy, but I still can't seem to get back to that weight and end all these annoying issues.

I'm not sure what could be the cause of all this, if the rectal discomfort and pain might also have something to do with me having to urinate all the time (which is anywhere from 2-5mins on a really bad day, to every hour or 2 hours normally), since the urologist did say my rectum and prostate were very tender. And my increased heart rate at random moments scares me.

Anyone have any clue what might be at the root of all this, or what condition I might have??

I've started eating healthier recently, which leads to me being able to defecate daily so that isn't so much a problem anymore.
 
Chronic prostatitis has been associated with Celiac Disease, an autoimmune disorder of the small intestine. Given your history of intestinal symptoms you might want to get screened for it, even if it's just to rule it out, it's a simple blood test called the Celiac Panel.
