Most annoying movie cliches

What are your most annoying movie cliches? The obvious ones must be the baddie about to kill the goodie but before hand must reveal his plan.

And also when there is a large shootout and the good guy has no weapons and seems to be beaten the bad guy is suddenly out of ammo.

Bad guys with plenty of ammo in their machine guns failing to hit the good guy (whilst their bullets ricochet off the surrounding metal walkway)

The baddies being English (add to this a large slice of historical inaccuracy if the film is directed by or stars Mel Gibson, or if the film in any way features slavery in its storyline)
