military dog tags.... need a bit of help asap?

First, off its in the wrong order. Second, there is no service number it's her social security number.

Last, First, MI
SSI
Blood type
Religion (No religion then it'll say N/A)

If they are joining then they will give her dog tags and they will figure out her blood type.
 
i'm buying my friend a military dog tag,

i know this is what is written on them:

Blood Group
Service Number
Surname
Initials
Religion

BUT, we dont know her blood group and she doesnt have a service number and she has no religion. is there anything else i can write instead of blood group, service number and religion?

This would be really helpful! THANKS
