MILITARY (ARMY): medical history at recruiter's vs at MEPS. difference?

At my recruiter's office, I filled out a medical form. The form has a laundry list of conditions that require a yes/no response. I had to put my initials after each response to verify the answer was correct. I have no serious conditions; my only "yes" pertained to vision. But, I had 2 small issues that I raised w/ the recruiter, and he told me to check "no": braces (for my teeth) and a hairline fracture. He said they'd only check on those things if I gave them a reason to. But my understanding is that I will eventually sign something that grants my permission to check my medical history. The braces and fracture are really no big deal; but they are DOCUMENTED. Now, if the military sees my medical history, those two things will show up and they'll think I was lying. So, my question is, will they also give me that form at MEPS? I will be completely honest at MEPS because I don't want them to get suspicious at me for lying about anything. If I fill out the form at MEPS, does the form I filled out at the recruiter's office hold no weight then? Or do they use it to start getting waivers for me (for my little fractures)? I was going to tell the recruiter that I want to change my answers to honest ones, but he'll probably get upset that I'm going against his advice. So, in a nutshell, is the medical form that I filled out at the recruiter's office important at all for waivers or anything, or is it just some sort of little "preliminary" that really means nothing at all? Will I also fill out that form at MEPS? Is it ok if the answers on the 2 forms do not match? (I'll just tell them that I wasn't paying attention when I filled out the form at the recruiter's office).
