MicroD or Hemilam+D which way to go?

I am a 58 year old male and have been physically active all my life, running, bicycling, hiking and backpacking. 6 weeks ago I got some sciatica on Saturday and by Monday I could not sit stand or walk without extreme pain and nurabness+tingling in the right buttocks, back of leg, calf, bottom of foot and toes. I had to lay on my left side in near fetal position for any relief. Week 1 got MRI showing L4-L5 protrusion and 1.5cm L5-S1 extrusion with distal migration 2cm down the canal impinging on the nerve root. Week 2 went to pain doctor, 1 epidural, vicodin script and
 
The fact that you have an extruded disc will probably mean you will have to have a hemilaminectomy and discectomy instead of a minimally-invasive form of surgery.

You could certainly try to find a surgeon who does minimally-invasive spine surgery for a 2nd opinion to see what they say, but based on what you're telling us you will need a surgery soon.

Also, as severe as your symptoms are, you are probably better off to have surgery sooner vs. later to prevent permanent nerve damage that can occur if you wait too long.
