medium hamburger at a restaurant undercooked?

At restaurants it seems that whenever you tell the chef to cook it one way, they always undercook it, if I order medium-well- to a cook that means medium, if I order medium- they cook it medium-rare. I used to order all my meat medium-well for this reason, but for the past year I've been ordering medium because it seems that everyone does this. Anyway, I ordered a hamburger tonight medium and to me it looked medium-rare- the inside was pink, but not bloody. The restaurant I think is pretty good quality, I used to get hamburgers from there a lot and was always fine. I know that you are supposed to cook hamburgers longer than steak for the fact that the meat is ground so it is exposed to bacteria all over. I'm just wondering how often people order hamburgers like this, and am I going to be okay from all the bacteria and stuff? If I did get e-coli or salmonella or something of that nature would I know right away, or approximately within the next few days? Also, how would I know if I was poisoned by this? Thanks
