Medical history journals for cats? O_o?

I am horrible at remember things. HORRIBLE! So, I was wondering if they have journals/books or something which I can keep medical record history about my 2 male cats? Such as vaccines, surgeries medications, their vets etc?? (preferably those that ask a Q, but leave a blank space so you can fill it in) if so, where can I buy one from?
 
i would definately like one too if you found one.
i have decided since now i will keep a record of my cat and cattens medical history because i have a very clumsy kitten who got ran over, had surgery, i have records of his meds, microchip cert, registration papers, photos (incase he goes missing and need a photo to put on signs), what they do/dont like etc
thanks for a good idea, i think im going to make a scrapbook of it and use his certificates and registration papers as a "birth certificate" of sorts.

You know, it seems like I got something like that from my veterinarian when I first brought my younger cat in, as a kitten - they had a whole welcome kit from Hill's Science Diet - a sample of kitten food, some information about kitten behavior, and a veterinary history that you could fill in with dates of vaccines and operations, etc. So, ask your veterinarian if they have such a thing.

Me, I've never been good about filling those things out. But I have noticed that the over the last ten years or so, that receipts from veterinary offices tend to be very detailed, including what procedures were done, medications given and the dosing instructions. So I have been keeping those all in a file together.

Also, when I moved, and had to find a new vet, the old one did not mind faxing the medical histories of my cats to the new vet. So if you can remember the name of your veterinary clinics or keep their contact information, you can get the info you need.
