mazda pick up front end vibration?

I have a 1999 Mazda B4000 4x4 P/U. Somewhere in the front end (motor, front drive shaft, ?) the truck vibrates. I know it's not the wheels or tires b/c the vibration stops immediately when I let off the gas and starts up as soon as I hit the gas again. At low speeds, it's not that noticeable, but from 55 to 65 it's intolerable. The change in the console jingles. It sounds like B-R-R-R-R-R-R-R-R-R-R. It feels like I'm driving on the rumble strip on the side of the highway. From 65 to 70 it mellows back out again to a tolerable M-M-M-M-M-M-M-M-M, but still annoying. By 75, it's back to the rumble strip. When I shift into drive or reverse, it clangs one time. When accelerating from a stop, it goes ting-ting-ting-ting..... It makes no noise or vibration when I rev the enging in Park or when I hold the brake and rev the engine in Drive. Acceleration is fine, either on flat ground or going up hill. Is is the front drive shaft, transfercase, what else might it be?
