Jeep Wrangler Transmission Problems?

I have a 2005 Jeep Wrangler (automatic). For the last year it's been acting up on me, and I don't know what it is. I'm thinking it's the transmission.

I don't know anything about cars so I'm going to try to explain it as well as I can, and I hope you guys can help.

Ok so say I drive my jeep going 65mph on the expressway for about 20 minutes without stopping. If I get off the expressway and have to slow down to about 30mph or have to stop on a stop light or stop sign, I can feel my Jeep jerk pretty hard as it drops a gear (i think that's what is called). When I try to speed back up at 50 or higher, it wont catch the gear (the RPM thingy). The faster I go the higher the the rpm needle goes. I have to put my jeep on neutral turn it off and start it up again, so that it will catch that gear again. If i slow down or stop it will do the same thing again.

The first time I got it checked I was told I just needed to chain my transmission oil, and I did but that didn't help.

Then they told me i needed to pour some additive in there, and that didn't work either. Could it be that I actually need to replace my transmission?
