I've written down a lot of information about a story I want to write but I'm not

sure how to make it a story? How can I use information that I've thought out and written down to write the story? It's so much stuff that I'm having a hard time figuring out what should happen where, and generally figuring out how to put the thing together. Also I'm not sure what form I should put the story into, a play, a screenplay, short story, novel, etc. Can you please give me some tips?
