Is there a wireless pad available for people with speaking impairments?

I was wondering if there is available somewhere some kind of a pad that could contribute help for for example stroke patients that can not speak. For patients that have had significant damage, and can therefore not type on computer or write with a pen.

The device that I am thinking about is some kind of wireless pad that has maybe 10 buttons with words or phrases that is connected to a receptor, preferably attached to belt or pants, which then can play a prerecorded message affiliated with the selected button. (phrases such as Yes, no.. etc). After the button has been pushed the receptor plays the message.

Preferably there would be a mechanism that allowes for recording your own words or phrases.

Anyone familiar with any such equipment?
