Is it fair to say that in the NYC car bomb attack, Homeland Security utterly failed?

The attacker was a man who'd gone to Pakistan several times to receive training, but Homeland Security never knew that.

That the bomber successfully planted a bomb, and that only by virtue of incompetent bomb building did it not go off.

That once a likely suspect had been identified (good work by the FBI here), the bomber was still allowed to board an airplane to Dubai despite being on a no fly list.

So other than the FBI quickly identifying him through some good investigative techniques, is it fair to say that nothing went right?
