Income tax question on T4A ?

Hi! I recently took a job as a rehab therapist (in December), working at numerous retirement homes in the city. I am payed by written cheque, with no deductions taken off at all. I received a T4A form for tax purposes for the 2008 year. Am i considered self-employed??? Where can i go to find out this information? And other information concerning self-employed issues, like tax deductions and so on. Because i'm not paying CPP or EI, am i screwed if i'm let go down the road or get pregnant? Any information would be greatly appreciated.
Should i even be getting a T4A or is my boss just lazy? I'd rather be an employee and pay CPP and EI so i can get maternity leave or unemployment benefits if it came down to that?
