In the beginning of the book my sisters keeper is the 3 yr old anna or kate?

Jodi Picout has a flashback of a 3 yr old trying to smother the other sister? I can't figure out if that was Anna trying to smother Kate or vice versa. Ive finished the book but still haven't came up with an answer?
