In reality , are insane asylums , the same as depicted in movies?

Will I walk into an asylum to find patients picking and eating strawberries from out of the wallpaper , shouting out in pain etc . Or are there patients in there who are seemingly normal , or who'd be like the guy/girl next door type ?
