The other night a friend and I went out for sushi. She had never eaten sushi before and although she enjoyed it, almost straight after she complained of stomach cramps.

That night she apparently had mild diarrhea and there was blood in the toilet. She's too embarrassed to go to a doctor (I did tell her to).

What could be wrong with her? She said she didn't feel sick in any other way. She's only 20.

A small amount of blood in the stool or in the toilet is normal if she had "food poisoning". Whatever she ate that irritated her bowels and digestive system probably caused small amounts of inflammation in her bowels and colon and a little bit of blood is expected if she was having diarrhea. Also diarrhea can irritate your anal area which can cause a little bleeding. If she is okay now and not having anymore bleeding she is probably fine. And just so you know, people should never be embarrassed to go to the doctors, that is what they are their for, they see that everyday, it doesn't phase them. I am a nurse and it is just like second nature to deal with issues like this. Nothing to be embarrased about! :)
