If there's an epic level of virus on my PC, is it sufficient to reinstall

only the OS or? should I wipe and restore to factory reset?

Already have malwarebytes. I run that weekly along with another detection program, and have Norton Internet Security Suite 2010 running continuously.

Definitely backing up my kids' photos and all my music and e-books.
