remember the name of author? Just few stanzas from here and there is all I remember
It starts something like this:

phoolon se jaa ke kehna, kharon se jaa ke kehna
haale wafa, wafa ke maaron se jaa ke kehna
yaaron ke dil ki baaten yaaron se jaa ke kehna
ettahad jindagi ke sachche payam le jaa
ae hind jaane wale mera payam le jaa

Kya hum bhula sakenge azdad ke watan ko
bikhari hui fiza ko bikhare huwe chaman ko
ganga ki dilkashi ko jamana ke baankpan ko
ab tak wahi hai rishta, dilli se lucknow se..
well, I'm forgetting the lyrics, but it was a favorite song of one of my professors, and he told me that the poet was some Muslim who migrated to Pakistan after partition. I so much loved the meaning and the painful depiction of the poet that I had written it without even understanding the full meaning of the song. But now I see that I have misplaced the paper.
Can someone help me find the complete poem?
