I need to prepare for boot camp and I'm not doing so well. Any suggestions?

From what I know, I'll be required to be able to do about 50 push ups in one minute or less, and 45 sit-ups in one minute or less. I have not kept up with ANY amount of exercising since high school (almost 5 years ago) which I'm now regretting, big time. I'm 5'7 and only about 142 lbs (always been kind of skinny) and I am obviously ridiculously out of shape. The sit-ups don't seem like they will be much of an issue, but the push ups... if you want a good laugh, watch me try to do 10! I can barely do 6, at least military style: hands no more than shoulder length apart and going almost all the way to the ground. My dad bought me "the perfect push up" and it almost seems harder with them. I'm assuming that only being able to do 6 isn't normal? I do as many push ups and sit-ups as I can until I feel like my muscles are going to fall apart (it doesn't take much) and then I go for a run which usually lasts about 20 minutes? I don't seem to be able to run any further each day, in fact some days I can't run as far as I could before. I know nothing, absolutely nothing about fitness so this could be normal, just keep in mind when answering this that you're going to have to speak to me like a child because I have no knowledge of things like this and I have no choice but to get it done - I'm leaving for Air Force basic training on Feb 16th and I MUST be in at least decent shape or it's going to be really, really bad. Is there a certain diet I should be following? Should I be eating certain things and at certain times of the day? I have no idea where to go, what to do, or who to talk to in order to get some good advice so I'm hoping somebody out there will be able to help!
