I have some questions for authors (prefer published)?

There are many things that nobody is able to tell me, so maybe you can. My biggest thing, is how do you format your page numbers when sending it to AGENTS (not editors, agents). Do you still put your last name and the books name in the top corner with the page number across it? Another one is, does a manuscript include dedications, "about the author", etc. Please refrain from answering with any of these: How to send to the agent, how to be published (It seems that everyone keeps answering me with just that). And if manus don't include those things, then when are you supposed to give it!? Thank you in advance
One more little thing, I know the page number should be away from the story, but would it be okay to move the story down in order to put it there?
 
No you make a cover page that is centered, double-spaced, 12 point font and type it like this!

Ex.

___________ <Name goes there

___________ <Number of Pages

___________ <Date Started

___________ <Date Finished

And as for the page numbers, format them in the top right hand corner of each page or you can go with bottom left, but never go with the other two not listed they are uncommon and hard to look for!
