I have a 1999 Ford Taurus...coolant cap keeps coming off leaking collant

everywhere...what is causing this? I have a 1999 Ford Taurus, at first we had problems getting the radiator fans working-we fixed it-now the fans work, but the coolant cap keeps coming off when I drive it, and I am leaking coolant everywhere! When I open the hood the coolant cap is off! I tightened it really good, but it stil comes off..any answers as to why? I am just curious-not expecting an overtheinternet fix
 
Hey ... i am having the same problem with my taurus 99. How did you fix the problem? Thanks
