I dont know how to do a clamidiyah test as i've lost the instructions, does anyone know?

I got the pack that the goverment is sending out to everyone between the ages of 18 and 25. I have the swab, a plastic wee container which has some orangey fluid in, a form to write my name and address on, and the return envolope, but i must have lost the bit of paper telling you how to do it.
