I can't remember the title or author of a book I read! Do you know this book?

It was about a girl who goes back in time to see her grandmother as a child her age. They look identical, and decide to swap places for a while. The grandmother see's herself in the future, and her face is warped from a car accident back in her original era. Oh, and the grandmother is in a coma in the future.
