This book was probably written in the 90's but I'm not completely sure. There were a group of people that were being trained in a virtual environment to be military commanders (?). They were never not inside this virtual environment, they didn't know what the real world was. They would train in various ways and every now and then they would have a skirmish of sorts where they would be commanders in giant life-sized virtual wars against each other.
One of the specific between skirmish training sessions I remember was quite like a video game. All the people had to get through a temple ruins (something like that) without getting 'killed'. As they killed enemies the weapons that they started out with would upgrade into better weapons, but if they got hit by the enemies their weapon would downgrade.
Another bit I remember is that their 'avatars' in this virtual world were completely smooth, hairless, no genitalia, but some of them somehow figured out how to mold genitals into their avatars because it felt like something was missing.
The last bit I remember was that near the end of the book they managed to break themselves out of the virtual world and awoke in the real world. They were all plugged in with a bunch of wires and stuff. I think one of them ran out of the building they were held in and his brain pretty much exploded because of some sort of control device that had been implanted, or maybe just because the real world was too much for them? I'm not sure.
I would love it if I could find out what this book was.
