My passport will expire this June 8, 2010. I am now hurriedly studying the application for passport renewal but I noticed that there are no fields for changed status and even adding my own children. I am using the C1 form. Do I need another form and/or attach my marriage certificate and the birth certificates of my 2 kids? I am of dual citizenship here in the Philippines. My wife and kids are Filipinos. Thank you.
