How would i file my grandmothers GM Pension 1099-r?

Im filing my grandmothers taxes... Question is, im using turbotax, its asking if its Qualified government, public, military pension or Qualified private-source pension OR none of these... which is it? Its thru fidelity for GM

Follow up question, Can i claim head of household since apparently she doesnt qualify, can i claim head of household with her living with me? Just curious
