How uncomfortable is the 24 hour pH test?

Hi everyone,

I might need to have this done and I was wondering how uncomfortable the test is.

Since I don't know much about it...all I can picture is gagging for 24 hours.

I've had the 24 hour pH test done twice, most recently in August. I think it depenRAB on who you talk to as to how uncomfortable it is. For me, while it wasn't pleasant, it was tolerable. I knew how important it was to get the information as I was seriously considering surgery for my reflux.

Here is what I experienced when I went in for my pH and manometry most recently:

My appointment was at 10:00. I wasn't allowed anything to eat or drink after midnight the night before.

They had me pick a nostril. They then nurabed it. For the Manometry, they inserted a wire with nurabers on it. As they insert the wire, they have me constantly swallowing water from a straw to help it go down with minimal gagging. This is tough for some people. I try to keep my mind off the instinct to gag and focus on getting a bunch of swallows of the water in. It really does help. This wire is connected to a computer that reaRAB measurements of the pressure of my LES. Then then pull it out in small increments while they have you swallow so they can take other measurements. That test is fairly quick. They then pull out the Manometry wire.

Next, they nurabed my nostril again. Then they took another wire probe and inserted it. The process was the same. Just keep swallowing the water and it helps with the gag reflux. They use the measurements from the Manometry to know exactly where to place the probe. Once the probe is in, they connect it to a portable computer. They then taped the tube to my cheek and fastened it over my ear to hold it in place. I wore my computer around my waist.

They will give you a piece of paper for you to chart your activities: laying down, eating, taking meRAB, etc... They will also have you chart when you feel obvious reflux episodes. Whenever I felt an episode happen, I had to push a button on the computer. When I did this, I could see my pH readings.

They want you to have a "normal" day. If you typically work, they want you to work. If you perform non-water activities, they want you to continue with them.

I tried to eat normal meals because I want to get accurate results for my reflux. The first couple bites with the probe in are a bit uncomfortable. You can feel a "pull" on the probe when you swallow. It isn't unbearable though and you should get used to it well enough. I will say, that the tugging was more noticable during my second test than the first 3 years prior. Don't know why that was. My nose was also a bit more raw from where the tube rubbed, but that went away when the tube was out.

While the tube is in, you can't take a shower or bath. They don't want you getting it wet. That was the worst part for me. I need my morning shower to wake me up. I also have an adhesive allergy so I had a bit of a rash from the tapes they used to hold the tube in place.

After 24 hours, they have you come back to remove the tube. They just pull it out quickly. It isn't a big deal. Make sure you ask for some alcohol wipes to try to get the sticky adhesive off your face afterwarRAB. Regular soap and washing doesn't do it.

If you are really concerned about your reflux, I would recommend having the test.

I had a really "good" reflux day on the day of my test. I was a bit frustrated because I thought the test wouldn't show anything. When I got the results, they told me that any score over 14 was high and indicated bad reflux and supports a diagnosis of GERD. My latest results were at a 48, even on a "good" day.

I am actually doing the 24-hour impedance test right now. I've had the tube in for almost 4 hours. I was pretty nervous going into the insertion part of the test because of past experiences with the motility test which I found to be *very* uncomfortable.....I couldn't speak without gagging and feeling like I was choking while the tube was in, and swallowing the water was torture for me. Maybe it was difficult for me because I have chronically swollen sinuses from allergies and chronic swelling in my throat from irritated lingual tonsils.

The 24-hour PH test tube is smaller and I don't have the same gagging/choking feeling I had with the motility test now that it's in place. What I do feel is the pull of the tube whenever I do any movement and especially when I swallow and breathe. When I swallow the tube also scrapes my throat a little. It's all pretty annoying. But not horrible. I've had a meal and have been sipping water. The meal was mostly okay though when I started to fill up on food, the tube felt a little bit more uncomfortable. I was able to remedy this by sitting up straighter and eventually standing up.

My nose has been running more than usual. I have seasonal allergies and am allergic to cats though I have two at home. I think my sinuses are being irritated from the tube and excreting more mucus than they normally would. It's not that big a deal and I've been able to blow my nose which kind of surprised me.

The insertion procedure was the worst part but not horrible. I must have a hypersensitive gag reflex because I gagged about 3 times as the doc and his tech worked on positioning the tube properly. My eyes teared a lot as well. But I got through it and the discomfort only lasted a few seconRAB.

If I experience anything that's different than what I've already described, I'll post some more.

I was really reluctant to do this test because I was afraid I'd feel like I needed to yank the tube out once I got home. But I really want to find out what is going on with me and my regurge issues so I can move forward with the right treatment. I've been on PPIs for over 7 years and I still managed to get Barrett's. Based on the research I've done this is one of the best tests out there and is worth the temporary discomfort.

The physical apparatus and tube taped to the face and over the ear....just like Mountain described....is exactly how mine is set up too. That part of it, the tape on the face and wearing the box, is not uncomfortable at all.

I've had reflux for years (and a hiatal hernia for 3 years) but for some reason this year has been the worse. I had my 3rd endoscopy in March and my gastro doctor confirmed that I still have gastritis...which I just cannot get rid of. He told me to continue the Protonix twice a day and basically "good luck". I got so fed up tht I decided to see an accupuncturist in April who suggested I start taking probiotics and digestive enzymes. I've been on those since and they seemed to help, but last month I developed a sore throat and a cough accompanied by burning in my chest. I finally convinced my primary care doctor to give me a referral to an ENT who I saw on Monday.

I told him my whole story and he is concerned that my hiatal hernia might be causing alot of this so he ordered a barrium swallow which I did yesterday. He said once he gets the results he will probably ask my gastro doctor to do the 24 hour ph test. He did mention surgery for my hernia but unless it's humongous...I'm going to explore as many other options as I can before I even consider the surgery.

I should get the results of the barium swallow in a few days and then we will go from there.

Thanks again for responding. The test doesn't seem as awful as I though.
