How to write HTML code for a poll?

Not one of those websites that do it for you. I want to write the code.

I want the voter to have to write their name and e-mail address in order to vote, so that nobody can vote more than once. And the results to show, based on number of votes, which choice is the most popular.

Any help is appreciated.
