how to make a High Quality Image?

How can I make a High Quality Image on Photoshop? I use it a lot but I never actaully made a Highquality Image. as in , hardly any pixelation or blurryness or whatever. just a cystal clean image. from scratch.
 
Use a Canon 5D Mark II, Mamiya RZ67, Nikon D700, Leica MP or Hasselblad H3D. Find or produce excellent light and always use a tripod when you can.

Photoshop is just a post-exposure tool; think of it as a digital darkroom. A high quality image is produced by you and your camera. Nothing else.
