How to insert a draw box with html code in Mechanical Turk?

I am trying to make a HIT on Mechanical Turk with a box that people can draw an image in and then submit it. Every time I put a HTML5 canvas code in it returns the error message "Dhtml template Must contain a question". Could someone please provide me with a usable code to place in the design section of my survey.
